The COMPLETION PROCESS

⭐ Completion/Delayed Rating by the Registrar’s Office

Steps for application for completion/delayed rating from the university registrar’s office:

STEP A1: Fill out this ONLINE COMPLETION FORM: https://bit.ly/CompletionApplicationForm
for the terms:
🟢 Second Semester 2023-2024
🟢 First Semester 2023-2024

STEP A2: After filling up the form, the student shall get in touch with their faculty handler to complete any missed assignments/exams.  (click and read this link: Requesting Completion from My Subject Teacher)

STEP A3: The Program Head / Dean shall entertain completion for faculty members who are no longer connected with the University.

STEP A4: A student who gets an incomplete grade has two consecutive terms to complete it.

STEP A5: The Program Head / Dean will approve the granting of the completed grade.

STEP A6: Failure to get a passing grade for an incomplete grade after the elapsed of two terms would mean that the student will get a No Credit (NC) mark in the subject concerned.

STEP A7: A corresponding completion fee shall be applied:

🟢 for SGPS / Law students, 150 pesos per subject; and
🟢 for Tertiary students, 100 pesos per subject.


⭐ Requesting Completion from My Subject Teacher

Step B1: Download and fill out your subject’s grade calculator. Fill in all the yellow cells completely.

Step B2: Take a picture of your proof of payment of your completion fee (properly flattened on a flat surface, aligned, and rotated. Crop unnecessary background and/or other items)

Step B3: Identify your completion requirement(s) from the P3 grading period. Select one (1) option from below:

Option 1️⃣ If you have no activity or activities left to complete, write in your email, “I have no activities left to complete. I would like my grade to be processed with my existing set of activities for the completion.

 ▶ For this selected option, if your grade calculator is accurate, the grades (P1, P2, P2, and FG) indicated in the file will be used to process your completion

 ▶ You will no longer be submitting other activities and/or documents


 Option 2️⃣ If you have an activity or activities left to complete for a non-SSP subject (based on your grade calculator), state the activities you aim to complete within the allotted 1-hour completion schedule.

▶ Activities for completion are P3 activities which have the following time equivalents:

➡ Check Activities (except periodic exam, recitation, and milestone checking) = 30 mins each

➡ P3 Exam = 1 hour (45 mins exam proper + 15 mins buffer)

➡ Individual Standing Activities = 30 mins each

➡ Team Standing Activities = 30 mins each

➡ Team Milestone Activities (except Defense) = 30 mins each

➡ Defense/Redefense = 1 hour

➡ Permit/Proof of Payment = 0 min

➡ Completion Exam  (for 1sSY 2324 ITE 314 students who had presented final project only) = 1 hour (45 mins exam proper + 15 mins buffer)


Option 3️⃣ If you are applying for the completion of an SSP subject, you’ll need to accomplish all your missed activities based on your grade calculator.


🟢 Note: Completion time is set for only an hour and is not intended for the accomplishment of the bulk of the semester’s coursework for the class.

STEP B4: Send a completion request via email to mqcarpizo.up@phinmaed.com using your official UPang email with the following details:

🟢 Subject Line: Completion Application - Subject_Code Block
➡ 
eg. Completion Application - ITE 123 Blk 1

🟢 Message Content (using formal email message)

➡ State that you are applying for completion of the subject (state subject code, block, description) that you've enrolled previously (state semester and school year)
Include the following details in your email message:

1. Student full name and ID number
2. Identified completion requirements (based on Step B3)
3. Reason for delayed / non-submission of requirements
4. Document supporting your stated reason in No. 2 (attach this to your email as a file)
5. Solutions / actions to accomplish future requirements on schedule
6. Accomplished grade calculator (see Step B1; attach this to your email as .xlsx file)
7. Completion Fee proof of payment (attach this to your email as a .jpg file)


STEP B5: Wait for the reply. If no response is received within three (3) working days, you may send a follow-up email. Maintain a single email thread for your completion application.

🟢 Send formal emails in English.
🟢 Do not send follow-ups immediately after sending the email
🟢 No need to send a notification in the GC or PM that you've sent an email, especially during off-schedule hours. Emails have their own notification systems. 
🟢 Remember to attach documentary proof and grade calculator.
🟢 Completion application is accomplished individually (not as a team)

STEP B6: Wait for confirmation of submitted requirements.

🟢 If the requirements are complete, you may be scheduled for completion.
🔴 If requirements are incomplete, details will be provided and you’ll need to reply to the thread with the needed requirements.

STEP B7: Take completion and submit requirements.

STEP B8: The completion requirement is scored and returned to you.

STEP B9: Submit an updated grade calculator.

STEP B10: Wait for confirmation of finished completion with details of grades.
